15-'16 Forms Due
All School Forms for the 2015-2016 school year are due (Applied Music registration, Dace registration, LS Aftercare, US Parking Request, and Health). For Health Forms: access Magnus through myBerwick to upload forms and complete consents. Parent contact information must be edited in myBerwick and will automatically be updated in Magnus. US and MS students will not receive their fall schedules, cannot be permitted to attend sports pre-season practices, or attend classes before health forms have bee submitted. Current Berwick families can check the status and due dates of their forms on Magnus. Questions regarding health forms should be directed to the school nurse Karen Chiang.
